Payment and insurance

We currently accept most PPO, EPO, POS, Medicare, CHAMPVA, TRICARE Select, and employer-sponsored plans from these providers:

We are actively expanding our insurance coverage based on evolving demand. If your insurance is not currently covered, please let us know here so we keep it in mind as we grow.

We also accept self pay (cash) and HSA/FSA.

Most Haven Health visits are billed as one primary care visit. What you pay depends on how your insurance plan handles primary care costs.

There are three most common scenarios:

1. Flat copay

  • Your plan lists a dollar amount for primary care, like $30 or $45.
  • You pay that same amount at every visit, usually at the time of the appointment.

2. Percentage of the visit cost

  • Your plan lists a percentage, like 20% or 50%.
  • You pay that percentage of the visit cost, typically after insurance processes the claim.

3. Pay first, then copay

  • Your plan requires you to pay the full visit cost until a deductible is met.
  • After that, you switch to a copay or percentage.
  • These charges usually come as a bill after your visit.

For example:

You come in for a sore throat and STI testing and we bill your insurance $300 for these services.

1. Flat copay (shows $45 per visit) → you pay $45

2. Percentage of the visit cost (shows 50% of visit cost) → you pay $150

3. Pay first, then copay (with a $2,000 deductible not met) → you pay $300

How to find your copay:

1. On your insurance card: Look for PCP or Primary Care. You may see a dollar amount, a percentage, or nothing listed.

2. Online with your insurance provider: Check your benefits or coverage details under primary care visits. Member services can also confirm this quickly.

We offer transparent pricing for those paying without insurance. In some cases, you can pay for these services first, and then submit a receipt to your insurance provider for partial reimbursement.

To get to these prices, we look at comparable services in a market, healthcare costs, and also factor in inflation. Our goal is to remain affordable and accessible to the average patient, while ensuring our on-site teams are fairly compensated for the services they provide.

If your visit requires a co-pay, we collect it before your appointment starts.

Some services may generate additional costs depending on what happens during your visit. If anything affects your total, we will let you know and collect any remaining balance.

We accept Visa, Mastercard, American Express, Discover, JCB or UnionPay. We do not accept cash or checks at this time.

You will only receive a bill from Haven Health if your insurance plan only covers the partial cost of your visit. If you have questions about your bill you can call, text, or email us.

If you have a question about a bill from a lab (e.g., Quest Diagnostics), please contact the lab for more information. These costs are set between the lab and your insurance company, not by Haven Health.

Yes, we currently accept Medicare.

We do not currently accept Medi-Cal.
