Insurance we accept
We currently accept most PPO, EPO, POS, Medicare, CHAMPVA, TRICARE Select, and employer-sponsored plans from these providers (faded logos are in progress).
What you owe
What you owe is determined by your insurance plan. There are two types of costs. Haven Health costs (e.g., copays) and external service costs (pharmacies and labs).
Haven Health costs
Most of our services fall under one primary care visit cost. Your insurance plan will likely require one of the following:
- Pay a flat copay
- Pay a percentage of the visit cost
- Pay the full visit cost until a certain amount is met before paying a copay

External service costs
Labs and medications are billed separately by partner labs and pharmacies. Their contracts with insurers are separate from Haven Health, so we don’t control those prices.
You may receive bills from those services depending on your coverage.
