Temporary Insurance Disruptions

Dear Patients,

Due to the recent passing of Dr. O'Brien, there are some temporary disruptions with our insurance network.

Haven Health will temporarily not be in-network with:

• Anthem (we need to credential our new medical director)
• Blue Shield (we are awaiting our effective date)

For patients affected by these changes, we're offering discounted $75 self-pay visits to help you maintain continuity of care.
