Patient Operations Resume on 7/14

Dear Patients,

Thank you for your continued support and patience as we work to restore patient operations.

I'm reaching out with good news: we have secured a Medical Director and can resume patient operations with Jake Collins, PA-C, starting Tuesday, 7/14.

There are some temporary complications with our insurance network that we want you to be aware of.

Haven Health will be in-network with the following plans under Jake Collins' NPI: Aetna (starting 7/31), Cigna, Medicare, UHC, and subsequent Medicare Advantage plans.

Haven Health will temporarily not be in-network with:

Anthem (they do not credential directly with PAs)
Blue Shield (we are awaiting our effective date)

This gap will be resolved once our Medical Director is fully credentialed with these network plans. For patients affected by this change, we're offering discounted $75 self-pay visits to help you maintain continuity of care in the meantime. This network status only impacts whether visits with Jake are in-network for these plans. It does not affect our ability to prescribe medications, process labs, or imaging orders with your insurance.

What this means for you:

Patients can now book appointments with Jake Collins.
We will process and release test results for all patients, regardless of insurance.
Jake Collins's existing patients can request prescription refills through our standard protocols.
Patients with prescriptions issued by Dr. Jack O'Brien will need to schedule a new virtual appointment to have those prescriptions reviewed and re-issued under Jake's NPI. This is a legal and regulatory requirement because pharmacies cannot process prior prescriptions or refills associated with Dr. O'Brien.
